NDIS INFORMATION

Who Can Access Our Services

Your Care Support Services currently supports NDIS participants who are self-managed or plan-managed. We provide flexible and personalised supports for participants of all ages and abilities, helping them achieve their goals, build independence, and stay connected with the community.

We are also actively working towards becoming a registered NDIS provider, which will allow all participants, including those who are NDIA managed to access our services. Compliance, safety, and quality are at the heart of everything we do, and registration is an important step in maintaining the highest standards of care.

How Our Services Work

We tailor our supports to meet each participant’s individual needs, preferences, and NDIS goals. Our approach focuses on providing safe, reliable, and person-centred support at every stage.

  1. Initial Contact
Reach out to our team to discuss your support needs, goals, and how we may be able to assist.
  2. Plan & Support Discussion
We take the time to understand your NDIS plan, personal goals, routines, and preferences to ensure our supports align with what matters most to you.
  3. Comprehensive Personal Risk Assessment
To ensure the safety and wellbeing of participants and staff, we complete a comprehensive personal risk assessment before supports begin. This process helps us identify any potential risks and clearly outline support requirements, ensuring that appropriate strategies and supports are in place from the start.
  4. Service Agreement
A clear service agreement will be developed outlining the supports to be provided, schedules, and responsibilities, ensuring transparency for participants, families, and plan managers.
  5. Commencing Supports
Once everything is in place, supports can begin at a time that suits you, with flexibility to adjust as your needs or goals change.

Our goal is to make the process simple, transparent, and focused on delivering safe, high-quality support.
