We work alongside participants to support their interests, goals, and personal choices while encouraging independence and inclusion within the community.
Our support workers can assist with a wide range of activities, including:
• Attending social events and community activities
• Going to cafés, parks, or recreational venues
• Shopping and running errands
• Attending appointments or community programs
• Participating in hobbies, sports, or interest groups
• Visiting family and friends
• Building confidence in social environments
Our goal is to create opportunities for participants to build connections, develop confidence, and enjoy the activities that matter most to them, while always respecting their choices and independence.
DAILY LIVING ASSISTANCE
Your Care Support Services provides personalised support to assist participants with everyday tasks in the comfort of their own home. Our goal is to help individuals maintain their independence while ensuring they feel safe, supported, and confident in their daily routines.
We understand that everyone’s needs are different, so our supports are flexible and tailored to each participant’s preferences, lifestyle, and NDIS goals.
Our team can provide assistance with a range of daily living tasks, including:
• Personal care and daily routines
• Meal preparation and assistance with cooking
• Household tasks such as cleaning, laundry, and organising the home
• Support with establishing daily routines and structure
• Assistance with grocery shopping and home management
• General in-home support to maintain a safe and comfortable living environment

TRANSPORT
Your Care Support Services provides safe and reliable transport to help participants get where they need to go and stay connected with their community. We understand that access to transport can be essential for maintaining independence and attending important activities and appointments.
Our team supports participants by providing comfortable and dependable transport while ensuring they feel safe and supported throughout their journey.
Transport support can include assistance with:
• Attending medical or therapy appointments
• Travelling to work, education, or training
• Going shopping or running errands
• Attending social and community activities
• Visiting family and friends
• Accessing recreational or leisure activities
Our goal is to ensure participants have reliable access to the places and opportunities that support their independence, wellbeing, and everyday life.
SKILL DEVELOPMENT
At Your Care Support Services, we support participants to build practical life skills that promote independence, confidence, and greater control over everyday life. Our team works alongside participants to develop skills at a pace that suits them, focusing on their individual goals, interests, and abilities.
Skill development supports are designed to help participants gain confidence in managing daily tasks while encouraging long-term independence.
Our support workers can assist with developing a range of everyday skills, including:
• Cooking and meal preparation
• Budgeting and money management
• Grocery shopping and planning meals
• Communication and social skills
• Personal organisation and time management
• Building routines and daily living skills
